Health Care Power of Attorney and Living Will
Authored By: Allegheny County Bar Association
You have a fundamental right to make your own health care decisions. As long as you are well enough, you and your physicians will make these decisions. However, if some day you are unable to make those decisions about your care because you are injured or too ill, others will have to make these decisions for you. You have the right to choose the persons who will make those decisions for you and let them know your wishes about the treatment you want, and the treatment you don't want.
Plan ahead. Protect yourself. Protect your family.
The Allegheny County Bar Association provides this free form packet along with an instructional video to allow you to prepare legal documents making these important health care decisioins.
