Pennsylvania Petition for Modification or Contempt of a Custody Order

This custody form is for PARENTS only. This form will create a Petition for Modification or Contempt of a Custody Order that can be used to ask a court to change or enforce a custody order that is ALREADY IN EFFECT. You will be asked a series of questions regarding your custody situation and existing order.  After answering all the questions you can submit the answers and create a petition for YOU based on YOUR ANSWERS. This petition can then be printed and filed in your local court. PLEASE NOTE: This is NOT a fill-in-the-blank form.

IMPORTANT!  PLEASE NOTE - CRIMINAL RECORD AND ABUSE HISTORY NOW REQUIRED:

Beginning in September 2013 any person filing a complaint for custody, or any petition for modification for custody must also file a verification regarding any criminal or abuse history of the petitioner and anyone living in the petitioner’s household. This verification form is not included in the Pannsylvania Petition for Modification or Contempt of a Custody Order. What does my computer need to use these programs?
