Community HealthChoices Webinar - May 31

The Pennsylvania Department of Human Services (DHS) will be holding a free webinar on May 31, 2017 from 11:00 am-12:00 pm highlighting the upcoming “Community HealthChoices” program.
This program is part of the ongoing DHS Tips and Tools for HealthChoices Webinar Series.
HealthChoices is the Medical Assistance (Medicaid) program for Pennsylvania. In HealthChoices, you choose a primary care provider (PCP) and health plan for each family member.
- Your PCP is your doctor or other provider who gives you most of your regular health care.
- A health plan is a group of doctors, hospitals, clinics and other health care providers. They work together to meet your health care needs.
During this joint presentation Jennifer Burnett, Deputy Secretary for the Office of Long-Term Living in the PA Department of Human Services, and Kevin Hancock, chief of Staff for the Pennsylvania Department of Human Service's Office of Long-Term Living, will highlight the following aspects of the upcoming "Community HealthChoices" program:

- Readiness review
- Relevant timeline
- Consumer notifications
