Properly filled out, this optional form will satisfy the requirements of sections 201 (d), (e), and (g) and sections 301 (c), (d), and (f) of the Migrant and Seasonal Agricultural Worker Protection Act (MSPA). This forms also satisfies statutory requirements under section 11 (c) of the Fair Labor Standards Act (FLSA).
